In order to move the joint properly, it is rational to contract the inner muscle near the joint and then the outer muscle that exerts a large movement. It is desirable that the spinal and sacroiliac joints, which are the joint tissues of the trunk, become stable by contracting the pelvic floor and transversus abdominis muscle, and then activate the oblique muscles and the erector spinae to move. Repeated sports activities in these motor control insufficiency causes sacroiliac joint disorder, facet joint disorder, intervertebral disc disorder, and low back pain due to erector spinae muscle hyperactivity.
In addition, when moving the limbs, motor control that moves the limbs after stabilizing the trunk by the activity of the trunk muscles is required, and the dysfunction might causes not only lumbar disorders but also shoulder and elbow joint disorders. Acquiring the correct motor control function not only prevents these disorders, but also improves performance, so athletic trainers are required to understand this function.

Regarding low back pain (LBP), there are two types: i.e., specific and non-specific. The herniated nucleus pulposus is the representative of the specific LBP for athletes. The discogenic pain, type 1 Modic change and facet arthritis is reported to be the main pathology of the non-specific LBP. Once the pain generator is clarified, minimally invasive approach is possible to cure the problems. Full-endoscopic spine surgery is possible under local anesthesia with less invasive for the back muscle and it requires only 8mm skin incision; thus, it is suitable for athletes. HNP can be removed full-endoscopically, and the discogenic pain is treatable by the radio-pulse thermal annuloplasty. Very recently, full-endoscopic disc cleaning surgery has been applied for the type 1 Modic change. In conclusion, exact diagnosis for the pain generator and minimally invasive full-endoscopic surgery is the clue for complete return to the original activity for athletes.

The kinetic chain plays an important role in producing enough energy to play a sport such as throwing, batting, and kicking. Either or both of pelvic and  thoracic dysfunction is one of the most popular problems which cause sport injuries through segments of the kinetic chain.
In addition, function of pelvis and thorax is greatly related to trunk stability. The former constitutes the foundation of the core unit and the latter constitutes its roof, and so their dysfunction makes trunk stability decreased prominently. It is said that trunk stability is one of the most essential functions to prevent sport injuries, which plays a significant role in performing sport activity at a high level.
It is very difficult to evaluate the pelvic and thoracic function, because their movement is too small to measure either qualitatively or quantitatively. In this lecture, we will introduce the pelvic and thoracic functional evaluation which we devised and effective independent exercises to improve the pelvic and thoracic dysfunction.

Many reports have been made on the importance of core muscle activity in physical activity.Recently, it has been said that it is necessary to evaluate the trunk muscle motor control of each patient in clinical practice. We evaluate the patient's motor control in the trunk muscle group and upper and lower limb motor control by to applied resistance to the patient's upper and lower limbs in the four-point position, supine position, and prone position. In this symposium, we will report the research results by electromyographic analysis that clarified the difference in trunk muscle activity when the lower limbs are raised on four-point positions depending on the pelvic position. Introducing the actual evaluation and exercise guidance for patients with myofascial low back pain. Then, we will introduce the evaluation and exercise for patients with myofascial low back pain.

Low Back Pain (LBP) in Athletes: Effectiveness of Motor Control Approach in Conservative Treatment

Surgical procedures including full-endoscopic spine surgery are an “approach to structure”. Both the specific and the non-specific low back pain (LBP) in athletes are functional pathological conditions mainly due to mechanical stress in posture and movement. Thus, an “approach to function” that corrects posture and improves movement in order to reduce local mechanical stress is indispensable as a pre-stage for performing surgical treatment, which is an “approach to structure”. It is desirable that "approach to function" enables recover from LBP, improves function, and returns to sports, but if it is not possible to achieve return to sports, "approach to structure" such as minimally invasive full-endoscopic surgery should be indicated. 100% or more recovery for athletes can be achieved while aiming at "correction / improvement of function", which targets the main cause of pathological condition by "approach to function", coupled with "repair / improvement of structure" by surgical treatment. This is the introduction of a practical "approach to function" using the Motor Control Approach, including Pilates method.
